Medicare Advantage vs. Medigap: The Decision to Revisit Every AEP

Should you stay with Medicare Advantage or look at a Medicare Supplement during AEP? A plain-English comparison of costs, networks, and trade-offs.

Every fall I sit down with Midwest clients who ask the same question: am I on the right side of the Medicare Advantage versus Medigap decision? AEP is the natural time to revisit it, because your health, budget, and the plans available in your county all change year to year.

The core difference

Medicare Advantage replaces Original Medicare with a private plan that bundles hospital, medical, and usually drug coverage, plus extras like dental and vision. Medigap works alongside Original Medicare and pays most of the deductibles and coinsurance Medicare leaves behind. You add a separate Part D plan for prescriptions.

Where Advantage wins

  • Low or $0 premiums in many Midwest counties
  • Extra benefits like dental, vision, hearing, and gym memberships
  • One card and one plan to manage
  • An annual out-of-pocket maximum, which Original Medicare does not have

Where Medigap wins

  • See any doctor or hospital in the country that accepts Medicare, no network
  • Very predictable costs, often just the monthly premium
  • No referrals or prior authorizations for most care
  • Strong fit for snowbirds and frequent travelers

The trade-offs people miss

Advantage plans use networks and prior authorization. If you develop a condition that needs specialists, you may face delays or find your preferred hospital out of network. Medigap premiums rise with age and can be hard to get later, because in most states you can be medically underwritten if you apply outside your initial window.

When to revisit the decision

  • Your Advantage plan's ANOC letter shows big cost or network changes
  • You were diagnosed with a condition that needs ongoing specialist care
  • You are spending more time traveling or living in another state part of the year
  • Your Medigap premium has climbed enough that Advantage looks attractive again
